Contract  

​

These terms of engagement form the contract between the Dr Milli Consulting Ltd (trading as Dr Milli health) and the patient. They set out the boundaries of the relationship between the practitioner (s) and the patient.  

​

Terms of Engagement between Dr Milli health (“the Practice”) and the Patient 

 

It is very important that you read these Terms of Engagement and understand them completely as the Practice will rely on you having done so. Dr Milli health recommends that you print and keep a copy of these terms of engagement for reference.  

​

By using our Services in the practice, you thus confirm that you accept these Conditions and that you agree to compl. If you do not agree to these Conditions, you must not use our Services at the practice. We reserve the right  to amend these Conditions from time to time. Every time you wish to use the Services at the practice, we advise you check these Conditions to ensure you understand the terms which will apply at that time. 

 

About Dr Milli health’s service  

 

Dr Milli consulting Ltd (trading as Dr Milli health) is a company registered in England and Wales under company number 14958586  at our registered office at 23 Whitehill, Puddletown, Dorchester, England, DT2 8SB. We offer non-emergency private health services to adults (aged 18 years and over). 

​

 Our Doctor is registered with the General Medical Council and the Care Quality Commission. Our Doctor is subject to annual appraisal, and adhere to the process of continuing medical education and revalidation as set out by the General Medical Council. Our Doctor holds appropriate valid medical malpractice indemnity insurance with approved UK medical defence organisations.

Dr Milli health is a private medical doctor ‘Service’ that offers a wide range of services including private GP for chronic health disorders with the ability to do functional root cause analysis tests e.g basic blood testing, urine tests, stool tests and DNA genetic nutrigenomic testing. Dr Milli health combines and takes a whole person approach when assessing patients to deliver a holistic approach to healthcare. Dr Milli health aims to prevent, reverse and treat chronic disease and hormone dysfunction and look at how we can improve our longevity.

The key is treating each person as an individual and getting to the root cause of health problems. It’s a whole person approach and engages both the patient and the doctor in a therapeutic relationship.  

 

At Dr Milli health, we spend time with our patients listening to their histories and looking at the interactions of the genetic, environmental and lifestyle factors that can influence long-term health and complex chronic conditions. This offers a powerful tool for assessment, treatment and prevention of chronic disease. 

 

Our Doctor considers each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a ‘one size fits all’ approach. It must be noted that the outcomes from the nutritional changes may vary between patients with similar health problems and following similar nutritional programmes.

This clinic primarily deals with your hormonal and chronic health problems virtually online or via telephone. They can work alongside other medical professionals and health care professionals with your consent. Our standards of professional practice in medicine are governed by the GMC Code of Professional Practice. 

 

At Dr Milli health, we take into consideration all aspects of your wellbeing including your lifestyle. Lifestyle medicine is the application of 6 pillars: food, exercise, sleep, relationships, mental health and toxic exposure load to promote health and individual care. This type of care is applicable to those with chronic conditions, as well as those looking for support to enhance their health and well-being. 

 

Please note: Another doctor outside Dr Milli health may hold different clinical opinions on the same medical condition or issue and, provided these opinions are reasonable. You agree to hold Dr Milli consulting Ltd and the Doctor completely free of any liability under every circumstance relating to your initial consultation If this is the case.  

Our Doctor speak English. Our Services are not delivered in other languages. 

Our Doctor does not tolerate abuse or offensive behaviour towards our doctor 

 

Consultation/Appointment

You can book an initial appointment by visiting drmilli.co.uk and clicking on: Book consultation. It will be at the Doctor’s discretion as to whether such consultation request shall be accepted. 

​

Appointments are virtually online. You agree when arranging your appointment that you are based in the United Kingdom.  

​

You can contact the clinic directly to book an appointment on info@drmilli.co.uk if you need us to accommodate another date/time to suit your needs; we will try our best to accommodate this but there are no guarantees.  

You may only book a consultation if you are at least 18 years and live in the UK and are registered with a UK GP. We will confirm our acceptance of your appointment request by sending you an e-mail that confirms the appointment details. A contract will only be formed between us and you when we send this confirmation email. It is your responsibility to ensure that the patient details provided for the consultation are accurate and up-to-date. 

​

Clinic consultations shall be limited to the time slot selected at the time of booking the consultation. 

We will require verification of service users identity in the pre sent patent questionnaire and on the video consultation too. This will include but not be limited to asking to see  a government issued in date valid, photo identification (photo driving license/passport) during a video consultation. 

 

Fees

Please note that we undertake a review of our pricing each 3 months and prices may be subject to change. 

 

Payment for treatment must be settled before the appointment. We accept most major credit/debit cards via Stripe online and may with advance notice, accept bank transfer.  

​

Dr Milli health reserves to right to charge fees for administrative procedures such as change of prescriptions before follow up appointments, referral letters, liaison with third parties regarding your care.  

​

Any investigation (urine, stool sampling, blood pathology, DNA mouth swabs) costs and fees for the analysis of these tests are NOT included in the cost of consultation. Prices are available on request. The costs of these tests vary enormously depending on what is being tested and your bespoke needs. Patients are advised to ask the doctor at the time of consultation as to the likely cost of any tests being performed. Any tests done elsewhere and brought to consultation for interpretation, require a fee of £50-100 per test for interpretation.

Additional Fees may apply to administrative purposes outside consultation time & longer consultation time than booked. 

​

All fees where applicable will be payable by you under these Conditions are inclusive of amounts in respect of value added tax chargeable for the time being (VAT). 

​

The Patient Understands and Agrees to the Following/Your Obligations:

​

You agree to provide us with the information we need in order to help you. Any information you give to the Doctor must be in English and accurate.  

 
You agree that you will only access our services for you and not for any other person and follow the shared decision instructions you are given by our service and report any side effects of treatments we have advised you to follow. You agree to tell us if you are currently or have previously been investigated by another provider for the same or similar complaint whether within the NHS or at another private provider and disclose the results of this.  

 

All patients are encouraged to keep their NHS provider involved and are made aware that any risk and safeguarding information is communicated with the relevant people if required.  

​

You must be registered with an NHS GP for your routine and acute medical care. Dr Milli health is not an acute medical service. You consent to us contacting your GP regarding aspects of your case. If you do not consent, in your best interests and as s duty of care we can contact your GP.  

 

Prior to your initial appointment you should provide a basic GP patient summary and medication list which can be requested from your NHS Practice.  

 

If you are unclear about the shared plan formulated together you should contact info@drmilli.co.uk for clarification. 

 

You must contact the practice should you wish to continue any specified plan for longer than the original agreed period, to avoid any potential side effects.  

 

Recording consultations using any form of electronic media is not permitted  without the written acceptance & formal permission of both the Doctor and the Patient. 

 

If you are working with any other health care providers, or start to work with any other health care providers during your care, you will notify the practice.

You consent to your case being discussed anonymously with peer mentors to support your health plan 

You understand that while we are following evidence-based medicine and NHS guidelines as much as possible, we are also stepping outside this guidance and you consent to this approach. 

 

We may need to ask you for certain personal information in order to provide our Services. If you do not provide this information when requested, we may be unable to provide our Services. 

​

Investigations  

 

Note that all testing is an additional cost. 

 

We may charge an administration fee for the ordering, processing and interpretation of testing. 

Any laboratory test or diagnostic procedure is sent to one of our partner laboratories/diagnostic service. These partners are separate entities from us and to the extent permitted by law we disclaim any liability for any omissions from these partners. 

​

You understand the clinic or the place where the investigation is being arranged, will require payment upfront and prior to collection of any specimens Once the investigation is performed it is solely your responsibility to follow up the result with the Doctor during a follow-up consultation, arranged by yourself. 

​

You are not obliged to pay for any further tests but by not performing a test that is advised by Dr Milli health you agree to hold Dr Milli health completely free of liability under every circumstance arising from the initial consultation. 

In the circumstance that further investigations are advised by Dr Milli health but you are unable to or choose not wish to pay for it, you are requested and advised to see your NHS GP. You are aware that your NHS GP is under no obligation to undergo these tests advised by Dr Milli health.  

 

Emergencies and out of hours service 

​

Dr Milli health does not provide an emergency service or a service outside of clinic hours. She will aim to reply to email queries within a reasonable time frame but may be busy with her other roles & responsibilities. Any urgent issues will need to be discussed with your NHS GP. 

​

Dr Milli Raizada reserves the right to refuse access to her services. 

 

Dr Milli health has a zero-tolerance policy to any violent, abusive or threatening behaviour that is perceived.  

 

Dr Milli Raizada can only give you appropriate advice and treatment if she gets accurate and complete information about your health, so it is your duty to provide this to the best of your knowledge. 

 

Dr Milli health does not offer out of hours care and response to emails may take up to 5-7 working days. 

​

Treatment  

​

All assessments and treatments are performed by qualified, registered and insured practitioner who work within their defined scope of practice as dictated by their relevant governing body. 

​

You can expect to be treated with dignity and respect and not be discriminated against. 

​

You understand that no third party (including your NHS GP) is under any obligation to action any recommendation provided by Dr Milli health.  

​

You agree that if your doctor feels your care is best served by transferring it to the NHS or other private party, you consent to our clinics findings to be shared to all relevant parties for this to occur safely. 

​

You understand that Dr Milli health may refer you to a specialist directly on a private fee paying basis and if you have private health care insurance this may be covered under your policy. You understand that it is your responsibility to check with your private insurance whether the cost of further referral and investigation is covered under your policy. If you do not have private insurance, you can still be referred to a specialist on a private self-pay basis only. You will be solely responsible for arranging your private appointment with any specialist for further treatment. 

​

You understand and accept that in the event that a specialist referral is advised, and you do not want to seek private further care, you agree that Dr Milli health is completely free of any liability under every circumstance relating to your initial appointment.  

​

In the circumstance that further treatment and onward referral is advised by Dr Milli health but you are unable to or do not wish to pay, you are advised to see your NHS GP for consultation. You are aware that your NHS GP is under no obligation whatsoever to fulfil any referral, or any element of a treatment advised by Dr Milli health. 

​

In some circumstances Dr Milli health will advise the follow-up with themselves to monitor response to treatment or convey the results of investigation. You understand that you are not obliged to have this follow-up however by not doing so, you hold us and the Doctor completely free of liability for any circumstance arising from the initial consultation. 

 

Prescriptions 

 

When we issue a prescription, our intermediary system e.g Signature Rx will send you information for you to be able to take your prescription/paperwork to an available dispensing pharmacy nearby.  

​

You understand that the cost of the private prescription is not influenced in any way by us, or our Services and the cost of the medicine is unique to the fulfilling pharmacy. Furthermore, we are not responsible for the dispensing or cost of any medication. You agree that the dispensing of the medication remains the responsibility of the individual pharmacist. The issue and prescribing of a prescription is at the discretion of the Doctor.

You understand, agree and accept that any prescription given to you is for personal use only. If you lose a prescription, a copy may be reissued by the Doctor and may be subject to an additional fee. You accept that the Doctor has the right to refuse to issue a copy of the prescription if it is deemed inappropriate. You must keep any medicines securely and do not allow others to use them. 

​

Insurances 

​

Where the client’s fees will be met by private medical insurance, payment must be made upfront to Dr Milli health; it is the client’s responsibility to secure a refund under the terms of their insurance. Every effort will be made to assist this process. 

​

Clients are advised to check carefully the terms of their insurance to verify that such treatment is covered. 

In all circumstances patients are responsible for the payment of their treatment. 

​

Privacy Policy

​

Cancellation Policy 

​

All payments and necessary forms must be received at least 48 hours before your scheduled appointment. 

Failure to submit your payment or signed consent forms within this timescale will result in automatic cancellation of your appointment. 

 

If you cancel your appointment with more than 48 hour’s notice we will provide a full refund minus the card fees (3%). Alternatively, it is possible to rearrange your appointment time at no extra cost if 48 hour notice is given.  

Should you cancel your appointment within 48 hours ofyour booked consultation time will forfeit any consultation fees paid. 

​

If you are more than 10 minutes late for an appointment, you will lose your appointment time and forfeit the cost of the consultation. Furthermore, you understand that if you are late for an appointment by a period of less than 10 minutes, the Doctor is not obliged to see you and you may still lose your consultation fee. If Dr Milli health can still accommodate your appointment, it will be at a time that suits Dr Milli health at their discretion, and should this not be acceptable, you will lose the fee. We will try to be as flexible as possible. 

​

You hold Dr Milli health completely free of liability under every circumstance relating to your reason for initial consultation in the event that you are late, and your appointment is cancelled in any eventuality.  

​

You accept that due to the nature of healthcare, we cannot guarantee that an appointment will necessarily be kept at the exact time booked appointment. You understand that there can potentially be a wait time of 30-60 minutes minutes for your appointment and once this time has elapsed, you have the option of either rebooking or receiving a full refund without the planned consultation.  

​

Without limiting its other rights, Dr Milli health may cancel an appointment by giving you twenty-four hours’ notice which may be sent via email. 

​

Without limiting our other rights, we may terminate any contract or cancel any appointment with immediate effect by giving written notice to you via email.  

​

Additional Terms

​

Discovery Calls

  • Discovery calls that are booked and not attended without prior notice may incur a £40 charge at the discretion of Dr Milli health. 

Mailing List

  • By booking a consultation, discovery call, or any clinical interaction, you agree to be added to the Dr Milli health mailing list. You can unsubscribe at any time. 

Transcription 

  • Please note that Dr Milli health uses Heidi AI for transcribing consultation notes and clinic documentation. Heidi AI is compliant with DTAC (Digital Technology Assessment Criteria), DSPT (Data Security and Protection Toolkit), and ISO certifications. It also undergoes CREST-approved penetration testing and is overseen by NHS-accredited Clinical Safety Officers. Transcripts are used solely for clinical care, documentation, and improvement of service quality. 

Testing Policy

  • All tests, including but not limited to blood, stool, urine, hormone, or DNA testing, once paid for, are final and strictly non-refundable. 

  • If tests are not arranged or collected by the patient within 6 months of the date of payment, the testing will be considered forfeited and no refund will be issued. 

Data Usage and Outcome Monitoring

  • Dr Milli Health processes all personal data in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. 

  • Patient data is stored securely with robust audit trails and access controls, and is used primarily for the provision of safe, effective clinical care. 

  • Where applicable, anonymised and aggregated clinical data may be used to assess service outcomes, inform quality improvement processes, or contribute to educational materials, research or reporting, in a way that does not identify individual patients. 

  • You have the right to request access to, correction of, or restriction on the processing of your personal data at any time in line with our Privacy Policy.
